·      “ACCOUNTABLE CARE ORGANIZATION (ACO)"
         Presenter: Jennifer DeCubellis, Hennepin County

Hennepin County’s integrated health care delivery network is designed to serve the unique needs of one of the most challenging and costly segments of the county’s safety net population. By integrating medical, behavioral health, and human services in a patient-centered model of care, the project seeks to improve health outcomes dramatically and lower the total cost of providing care and services to this population.
In this session, you will hear the various systems and how they have been brought together to improve health and reduce costs.

1:15 - 5:00 P.M.              MINNESOTA ASSOCIATION OF COUNTY ADMINISTRATORS (MACA) PROFESSIONAL DEVELOPMENT SESSION

 

1:15 - 5:30 P.M.              ‘TRANSIT VISION’ BUS TOUR

This “On the Road” educational coach bus tour specifically designed for County Commissioners will feature several metro area transit projects, from operating lines to drawing board proposals. (See tentative schedule)

·         Gain a back-room view of the restoration work at the historic Union Depot in St. Paul.

·         Witness station construction along the Central Corridor Light Rail Transit route.

·         Visit Target Field and learn about the new multi-modal transit hub, The Interchange.

·         Step aboard the Northstar Commuter Rail, and ride the Hiawatha LRT. 

·         Enjoy a rare look inside the LRT maintenance facility. You might get a look under the hood of LRT vehicle.

All along the tour, hear from fellow commissioners on how county leadership is making these projects happen.
